The Glasgow Coma Scale (GCS) is universally used to assess conscious level in the acute phase of brain injury and should be charted as a graph to enable easy identification of a change in the patient's condition. It is more accurate in assessing altered levels of consciousness due to cerebral trauma than medical causes of coma.
